The formal proof said all_prior_failures_retained was true without
qualification. The first a073ccb derive run (its two outputs, stdout
and first script bytes) and the two stage-only UI diagnostic copies are
not kept as original bytes, so the field is now false. New fields state
what is kept (every formal test raw output, the earlier reviews and
deliveries, the failed run logs and receipts) and what is missing, with
a correction note; record_gaps_kept is unchanged. The ledger references
to the proof take its new hash and the static plan check is rewritten.
The review result, delivery, raw outputs and history are unchanged.
