The r03 S f651ce1 failed its formal OPT verification: the save sample of the migration
attempt of a rejected out_of_range load stays in K. r04 extends decision D1 to that sample,
adds only that statement to the SPEC section 2 sites and derives the fixed table again from
the before record only. Docs only; the r03 task keeps its failure.

Docs only. Adds execution/p28-02/opt/: SPEC.md, TASK_INSTRUCTIONS.md and
registration/ (acceptance baseline of 12, registration and config
proposals, the before-derived fixed table of allowed cross-version
differences, and the byte-transfer record). Decision D1 is option B:
only the load_from_path failure path stops recording its performance
telemetry; the frozen E3 rule stays. The improvement rule is the formally
completed EQ SPEC §4. No product, frozen EQ or original-condition change;
23/88 and 4/30 unchanged. Registration is Root's; implementation is Opus.

P28-02-EQ (support task) completed through the normal Runner flow:
S 9adbbee -> Q 4e34e52 on main 54e07ca. Exact-S native 7 phases,
formal unit 179 and native-verify, fresh independent Opus review of all
6 conditions (pass, low 2), normal integration and final, joined with
the actual Runner DB and traceability. The earlier changes_requested
(a46418d, EQ5) and the b2dbda8/6f75c9a failures stay preserved.
Original P28-02 remains in progress; 23/88 and 4/30 unchanged.
